Does Coffee Cause Hair Loss — and Does Caffeine Shampoo Work?
Two different questions get mixed together whenever coffee and hair come up. One is whether drinking coffee makes you lose hair. The other is whether caffeine applied to the scalp helps hair grow. The evidence points in opposite directions, and the honest answer to each is short.
The short answer
Does coffee cause hair loss? There is no clinical study linking coffee consumption to androgenetic alopecia or to any other form of hair loss. Nothing in the published literature supports the idea.
Does caffeine on the scalp help? Laboratory work on human hair follicles is consistently positive, and one randomised trial found a 0.2% caffeine solution not inferior to 5% minoxidil at six months. But the clinical evidence as a whole is thin and mostly low quality, so caffeine belongs in the “reasonable to try” category, not the “proven treatment” category.

Does drinking coffee cause hair loss?
No published clinical study has examined coffee drinking as a cause of hair loss and found one. The literature on caffeine and hair is almost entirely about caffeine applied to the scalp, not swallowed.
The idea persists for two reasons. The first is that heavy coffee drinking often travels with the things that genuinely do affect hair — poor sleep, chronic stress, skipped meals, crash dieting. Any of these can push follicles into telogen effluvium, a diffuse shed that shows up two to three months after the trigger. Coffee is visible in that picture, but it is not the cause.
The second is timing. Most men notice thinning in their late twenties and thirties, which is also when coffee consumption tends to be highest. Androgenetic alopecia is genetic and hormone-driven; it would have arrived on the same schedule with or without the coffee.
If you are losing hair and drink a lot of coffee, the useful thing to check is not your caffeine intake but your ferritin. Iron deficiency is a documented, correctable contributor to hair loss and is easily missed.
What caffeine actually does to a hair follicle
The scientific interest in caffeine started with a 2007 organ-culture study. Hair follicles from 14 biopsies taken from the vertex of men with androgenetic alopecia were cultured for 120 to 192 hours. Testosterone at 5 µg/mL significantly suppressed their growth — and caffeine at 0.001% and 0.005% counteracted that suppression. Caffeine on its own significantly stimulated follicle growth, confirmed on Ki-67 staining.
A larger 2014 study extended this to female follicles and to the signalling behind it. Male and female scalp follicles were cultured for 120 hours with testosterone alone or with caffeine. Caffeine lengthened the hair shaft, prolonged anagen and increased matrix keratinocyte proliferation. It suppressed TGF-β2, which drives follicles into catagen, and raised IGF-1, which keeps them in anagen. Notably, female follicles were more sensitive to caffeine than male ones.
These are real, reproducible findings — but they are findings in dishes. A follicle in organ culture is bathed in a controlled caffeine concentration for five days. A scalp is not.
Is caffeine a DHT blocker?
No, and this is the most common misunderstanding.
Finasteride is a DHT blocker in the literal sense: it inhibits 5-alpha-reductase, so less testosterone is converted into dihydrotestosterone. Caffeine does nothing of the kind. It does not inhibit that enzyme and does not lower DHT.
What the laboratory work shows is different: caffeine counteracts some of the downstream effects of testosterone on the follicle, by reducing TGF-β2 and raising IGF-1. The follicle is still exposed to the same androgen; caffeine appears to make it slightly less responsive to the growth-suppressing signal. Useful, potentially — but not the same mechanism, and not the same magnitude. Anyone selling caffeine as a DHT blocker is describing something the research does not say. Does caffeine shampoo work?
This is where the actual clinical evidence sits, and it is a mixed picture worth reading carefully.
The strongest single result. A 2017 open-label randomised multicentre trial in 210 men with androgenetic alopecia compared a caffeine-based 0.2% topical liquid against 5% minoxidil solution over six months. Measured by trichogram, the anagen ratio improved by 11.68% with minoxidil and 10.59% with caffeine — a difference of 1.09%, which met the trial’s non-inferiority criterion.
That headline deserves three caveats. The trial was open-label, so neither patients nor investigators were blinded. The endpoint was the anagen ratio on a trichogram, a surrogate measure, not a photographic hair count. And the study was industry-conducted, by a manufacturer of caffeine hair products.
The whole body of evidence. A 2025 systematic review searched PubMed, Scopus and Web of Science for clinical trials of topical caffeine products through November 2024. Of 1,121 articles screened, 9 met the inclusion criteria, covering 684 people. Every one of the nine concluded in favour of topical caffeine. But when the reviewers graded the evidence, quality was medium in 3 studies, low in 1 and very low in the remaining 5. The recurring flaws were missing randomisation, missing placebo and control groups, and products that did not state their caffeine concentration. The reviewers’ conclusion: topical caffeine preparations appear safe and effective, better trials are needed, and commercial caffeine products “may be worth a try”.
A separate 2025 review of investigator-blinded controlled trials of non-prescription treatments for androgenetic alopecia included caffeine among 13 studies of eight agents, and reached the same place: measurable improvements in hair density or diameter, minimal adverse events, and a need for larger, better-designed trials.

Why contact time matters more than the brand
If you use a caffeine shampoo, the single thing that decides whether the active ingredient reaches anything is how long it stays on your scalp.
A pharmacokinetic study measured what happens after a caffeine shampoo is left on for two minutes. Caffeine penetrated through the hair follicles faster and in greater quantity than through the surrounding skin, and the follicles were the only route for fast caffeine absorption during the first 20 minutes after application.
Two practical consequences. First, a shampoo rinsed off in fifteen seconds delivers very little; the products tested in trials were left on. Second, the delivery route is the follicular opening itself, which is why caffeine is worked into the scalp rather than through the lengths of the hair.
Caffeine serums, and rinsing your hair with coffee
Leave-on caffeine serums have the theoretical advantage over shampoo: contact time is not limited to a wash. There is no trial comparing a caffeine serum against a caffeine shampoo, so the advantage remains theoretical.
Rinsing hair with brewed coffee is a different matter. No clinical trial has tested it. Brewed coffee is not a standardised preparation — concentration varies enormously and nothing in it is formulated to reach the follicular opening. It can temporarily darken light hair, which is probably where some of the anecdotal reports come from. It is harmless; it is simply not evidence-based.
How much coffee is too much for your hair?
Since no amount of coffee has been shown to cause hair loss, there is no hair-specific limit to give you. The sensible limits are the general ones — sleep disruption, anxiety, palpitations — and they apply to your whole body, not your scalp.
Where coffee intake does become worth examining is when it replaces meals or disrupts sleep badly enough to matter. Both routes lead to the same place: a diffuse telogen shed, months later, that looks alarming and resolves once the underlying pattern is corrected. That is a nutrition and sleep conversation, not a caffeine one.

The question changes after surgery, and for a different reason: caffeine is a mild vasoconstrictor and a diuretic, and the first days after a transplant are when bleeding risk and hydration matter. What to use instead, if you want something that works
Caffeine is not a substitute for treatments with strong evidence. For androgenetic alopecia the established options are minoxidil and, for men, finasteride, both of which have far more and far better trial data than any cosmetic ingredient. A caffeine shampoo can sit alongside them; it does not replace them.
Where hair has already been lost and the follicles are gone, no topical restores it. That is the point at which surgical restoration becomes the question — and the honest way to find out which category you are in is an assessment, not a shampoo.
