Miniaturization is a follicle shrinking, not a hair thinning. The same follicle keeps cycling, but each new growth run is shorter than the last, so the hair it produces comes back finer, shorter and paler every time. Nothing falls out permanently at first — the hair is simply replaced by a smaller version of itself.
The mechanism is a shortening growth phase. Androgen receptor activation progressively cuts the length of anagen, and a shaft that has less time to grow ends up thinner and shorter (Ho, Sood & Zito, Androgenetic Alopecia, StatPearls). Repeated over years the follicle ends up producing something closer to body down than to scalp hair.

It is the earliest measurable sign of pattern loss. Long before anyone sees thinning, a scalp already shows a mixture of thick and thin shafts side by side in the same area. That mixture is what magnification is looking for, and it is why a diagnosis can be made from a scalp that still looks full.
It also decides where grafts may be taken. A donor area with miniaturizing hairs is not a stable donor, however dense it looks — those follicles keep shrinking after they are moved.
